Job Summary
MercyOne Home Care- Des Moines is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need to maintain their health and independence.
Key Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Administer medications and treatments as prescribed and monitor patient responses.
- Educate patients and families on disease management, medication usage, and health maintenance.
- Monitor vital signs and manage chronic conditions, ensuring timely interventions.
- Document patient care activities accurately and in a timely manner.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Provide emotional support and counseling to patients and families.
- Ensure compliance with all healthcare regulations and standards.
- Participate in quality improvement initiatives and ongoing education.
- Current Registered Nurse (RN) licensure in the state of Iowa.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) acceptable.
- Previous home health or community nursing experience preferred.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation.
